Alpena Gymnastics Waiver

AGREEMENT REGARDING RISK OF INJURY AND RELEASE AND WAIVER OF ALL CLAIMS:

I hereby apply to have myself/son/daughter participate in the Alpena Gymnastics, Inc. program. I understand that participating in the program will expose myself/ my child to above normal risks of injury or harm. I understand that tumbling and use of gymnastics equipment may be a hazardous and may result in injury.

I represent that myself/ my child has no health or physical problems that will interfere with my/his/her participation in the gymnastics program. I agree that I will not hold Alpena Gymnastics, Inc. responsible for me/my child’s safety.

I hereby and forever release, discharge, and agree not to sue, Alpena Gymnastics, Inc. for any or all claims, causes of action or liability for any injury, loss or damage sustained or incurred in any way associated with me/ my child’s attendance at our participation in the Alpena Gymnastics, Inc. program.

I have read the above items of the Release and Waiver, understand them, agree to abide by them.
